You can use your practice's client portal to complete a form that you received in an appointment reminder.

1. Select the appointment link or button that shows in an appointment reminder.
   
   Vello opens the client portal.
2. In the home page, select Complete Forms for the applicable appointment.
   Note: It is possible that Vello does not show Complete Forms but does show View Appointment Details. If you select View Appointment Details, you can get access to the available forms in the Appointment Details screen.

3. In the Forms section, select the applicable form and enter the necessary information.
   
4. Select Complete.
